Root Cause Analysis (RCA)

The Root Cause Analysis (RCA) is a method of problem solving used for identifying the root causes of faults or problems. A RCA is the tools, and techniques used to uncover causes of problems to help identify what, how, and why an event occurred so that steps can be taken to prevent future occurrences. A RCA is all fundamentally connected by three basic questions:  What’s the problem? Why did it happen? and What will be done to prevent it?
